1. Go to www.tourismboracay.com to submit an online health declaration form.

2. The following requirements must be e-mailed to touristboracay@gmail.com:

a) Negative RT-PCR COVID-19 test taken within 72 hours prior to time of arrival

b) Booking confirmation, which must be with a hotel accredited by the Department of Tourism (DOT)

c) Online health declaration form confirmation

d) Round-trip tickets

e) Valid ID
